What Is Diabetic Retinopathy?

Diabetic Retinopathy is a vascular disease occurring in the retinal vessels due to long standing & uncontrolled diabetes. If left untreated, it may lead to severe loss of vision. With timely diagnosis and proper treatment the vision can be saved.

Symptoms

  • sudden and total loss of vision
  • blurred vision
  • floaters, or spots and dark strings that float in the patient’s field of vision
  • patches or streaks that block the person’s vision

Causes and risk factors

  • Long standing diabetes
  • Uncontrolled blood sugar levels
  • Coexisting Hypertension
  • Deranged lipid profile
  • Smoking

Management

Diagnosis of diabetic retinopathy can be made on routine fundus examination. However certain investigations are done periodically to assess the severity and grade of disease. These include :

  • Fluorescein angiography
  • OCT (macular scan)

Treatment depends on the grade of retinopathy and includes :

  • Strict control of blood sugar and BP
  • Intravitreal injections of anti-VEGFs
  • Laser photocoagulation
  • In advanced cases with complications surgery in form of Pars plana Vitrectomy maybe required.

What Is ARMD?

Age related macular degeneration is one of the leading cause of blindness worldwide. It is a disease of elderly age group seen usually after 60 years. There are 2 types of ARMD :

  • Dry type – characterized by drusens and usually leads to gradually progressive mild to moderate loss of vision
  • Wet type -characterized by development of CNVM (choroidal neovascularization) and may lead to sudden and severe loss of vision.

Clinical presentation and diagnosis

Diagnosis of ARMD is usually clinical. The affected patients are elderly, with gradual or sudden decrease in vision in one or both eyes. After dilated fundus examination the typical findings suggestive of the type of ARMD can be seen.

Few investigations required to assess the severity and grade of disease and also the response to treatment are :

  • Fundus photography
  • Fluorescein angiography
  • OCT (macular scan)

Treatment of ARMD

The treatment depends on the type of ARMD.

  • For DRY type the only option is multivitamin antioxidant capsules. These are prescribed according to the recommendations of AREDS study, which suggest their long term use prevents progression of disease.
  • For WET type disease we have intravitreal injections of anti-VEGFs like Bevacizumab (Avastin), Ranibizumab (Lucentis, Razumab). These injections have to be given every month for atleast 3 months and may need to be continues even further depending on response. The success of treatment is assessed by improvement in vision and also on OCT analysis.
