Common Types of Cataracts

  • Senile Cataract – as the name implies, this is seen in old age and is the most common form of cataract.
  • Congenital: Occurs in few children. May be a result of some genetic disorder, maternal malnutrition or birth trauma. Needs surgery at the earliest to restore normal vision in child.
  • Secondary: Occur in the presence of a situation such as diabetes, drug, and alcohol abuse or exposure to toxic substances such as radioactive rays and steroids.
  • Traumatic: As the name implies, these happen as the result of an eye injury and in some rare cases can be caused by electric shock.

Symptoms

    • Problems with glare during the day
    • Double vision in the affected eye
    • The trouble with eyeglasses or contact lenses not working well
    • The vision that’s cloudy, blurry, foggy, or filmy
    • Nearsightedness (in older people)
    • Changes in the way you see color
    • Problems driving at night

Cataract surgery

This is one of the most frequently performed surgical procedures in the world. The surgery involves:

  • Administering a local anesthetic to the eye(s)
  • Making a tiny 2–3 mm incision in the outermost layer of the eye (the cornea)
  • Making an evenly round incision in the ‘bag’ (capsule) containing the affected lens
  • Breaking up the affected lens into smaller pieces with an ultrasound
  • Removing these pieces with a vacuum and flushing the lens capsule to clean it
  • Implanting an IOL in the eye

Laser cataract surgery

The latest advance in cataract surgery is the use of a femtosecond laser to total some of the steps that the surgeon would otherwise perform manually. This is performed in the laser suite and contains the opening incision in the cornea, cutting into the capsule containing the affected lens and breaking up the lens for removal. The remaining steps are completed in the operating theatre.

Using a pre-programmed laser may reduce the risk of complications during and after the surgery, and some studies support the benefits of using laser cataract surgery where possible. In particular, people with other eye circumstances or complex cataracts may be more suitable for laser cataract surgery.

Different types of IOLs

Monofocal foldable lens – gives clear distance vision but requires glasses for near work.

Toric lens – corrects astigmatism and cataract both at one time to give good distance vision but glasses are needed for near work.

Multifocal / Trifocal lens – gives good distance and near vision with no or minimal dependence on glasses.

Cataract is the clouding of the lens that is seen most commonly in older age. Sometimes cataract develops even in younger age due to certain factors like trauma, diabetes, chronic eye inflammation etc. Rarely this condition can be seen in newborn children and it needs to be diagnosed and managed early for proper visual development of the child.

The standard treatment of cataract nowadays is phacoemulsification surgery. The timing of surgery depends on the patient’s visual needs and not related to the maturity of cataract. Patients can choose from various different types of IOLs which provide good clear vision.
